I wish to apply for the permanent/full time position as a Registered Practical Nurse in hopes of becoming a member of your working healthcare team. As you will see from the enclosed resume, I have been working with and for people, young and elderly, for the beginning steps in my professional and personal life. I have currently moved to the North York area of Toronto, after leaving my home town in British Columbia and after working 2.5 years at a long term care facility; Kitimat Multi-Level Care, and working 8 months in a Long Term Care Facility in Kelowna, British Columbia. I enjoy my job, however I am looking to further my education and work experience here in Ontario.
My academic background includes graduating grade 12 from Mount Elizabeth Secondary School and a certificate in Practical Nursing from Northwest Community College. I have also obtained my License and Registration; both in British Columbia and here in Ontario, in Practical Nursing and am looking forward to returning to school in the future to obtain my Registered Nursing Degree. My communication skills have always been a major asset to me and I take pride in my ability to work well with others in a caring, work related and team oriented manner. These skills will allow me to contribute to your healthcare team in order to produce the best possible care for the residents and/or patients of your healthcare facility.
I am currently in the process of switching my driver’s license to an Ontario license; however I do have a vehicle and am willing to commute. Practical Nurse Job Description: To work along-side an RN. Working both on the floor; aiding the care aides in resident’s personal care and making and serving breakfasts, then stepping off the floor to continue with my daily nursing duties which entailed; admission/discharge of permanent and/or respite residents. Resident wound treatments, catheterization also teaching proper catheter care to residents and fellow co-workers. Processing Doctors orders and corresponding day to day with Doctors and residents and/or patients family. Initiating and updating care plans, charting on pertinent information regarding changes with residents and writing out incident reports when incidents arose. Preparing and dispensing medications, initiating palliative care treatment, preparing bodies for the morgue and pronouncing deaths. Also, working at times as the Head Nurse or In-Charge Nurse on Night Shifts.
Care-Aide Job Description: Assisting residents; with a wide span of Dementia’s and physical ailments, in their everyday personal am and hs care and activities of daily living. Making and serving breakfast, giving resident’s baths, transferring from beds to wheelchair via appropriate transfer machines or pivot belts. Assisting residents in improving daily routine and working with each resident’s physical ailment and/or dementia to help sustain suitable activities of daily living and to promote independence to each resident no matter their dementia and/or physical ailments. Also included in the daily routine, is assisting nurses with input towards residents personal care plans, charting on pertinent information, communication with head nurse of any changes in residents and writing incident reports for incidents that may occur.
2006-2007
• Sutherland Hills Rest Home, eight months employed as casual staff, accumulating approx. 165hrs/month plus. My job allowed me to work along-side RN’s full scope. Processing Doctors orders, corresponding day to day with Doctor’s and residents families. Preparing and dispensing medications, doing dressing changes ranging from cuts and scrapes to ulcerations and surgical incisions. Updating care plans and “my day” were also day to day tasks that would be carried out.
2005
• Terrace View Lodge, Gerontology Practicum, 6 weeks (180hrs). This Practicum allowed me as a student to express the knowledge that I had learned in my books and in class and use it to help produce productive and efficient days for the residents of Terrace View Lodge. From dressing changes to giving out medications, getting residents up in the morning and accompanying them to their daily activities, feeding residents who needed assistance, bathing residents and carrying out palliative care.
• Clinical Work, three days at the Kitimat General Hospital, three days at the Prince Rupert Hospital and thee days at Mills Memorial Hospital in Terrace, BC. During this clinical experience, I learned how things rand within hospital settings as a practical nurse. I began charting, reading and transcribing Doctor’s orders, doing dressing changes, am care, hs care, pre-operative preparation, along with admissions and discharges.
• Practicum 3- Acute Care (5 weeks) 150/hrs. Experience of 3 weeks in Kitimat General Hospital and Mills Memorial Hospital in Terrace, BC. This practicum was the first transition from teacher oriented learning to hands on, taking over an actual hired LPN’s work load. Charting, admissions, pre-op and post-op care, giving medications, dressing changes and am and hs care were just among the few things experienced during this practicum.
• Preceptorship 4 weeks (185hrs) at the Prince Rupert Hospital. This was the final transition from student to practical nurse. I was assigned to an LPN situated at the facility and took over her work load. New experiences arose from checking charts on nights shifts to going to codes in the Emergency and dealing with more acute illnesses and disabilities.
2004
• 30 hrs of related work experience hours at the Kitimat Multi Level Care. I attended church with the residents, helped out with daily activities, accompanied and helped residents do daily exercises.
